Centrepay set-up (SA501)

Setting up your Centrepay payment. You complete and sign the paper SA501 Centrepay deduction authority yourself. A Drive Blue or partner organisation worker can help you fill it in — but you must sign it yourself.

Print the form first, then work down this page. Each step below tells you exactly what to write on it. Once you have signed it, Drive Blue uses your SA501 to set up your Centrepay deduction and keeps a copy for your record. You can also set this up yourself any time through your Centrelink online account via myGov.

Print your SA501 now

Open the Services Australia page, read the Centrepay Customer Deduction guide, then download and print the Centrepay deduction authority (SA501). Keep the printed form beside you and fill it in as you complete the steps below.

No printer? Ask your Drive Blue trainer or your support worker for a paper copy — blank SA501 forms are available at every session. You need to sign the form by hand — a clear scan or photo of the signed form is all Drive Blue needs to set up your deduction.

Your fee depends on the community/town where you complete your training — it appears below once you nominate your Community/town above. Write this exact amount on your printed SA501 at Part D – Q14.

Sign and date the SA501 yourself (Q17), then upload a clear scan or photo below so Drive Blue can set up your Centrepay deduction. A scanned copy is accepted — you keep the paper original for your own records.
